Job Description:
The Case Manager will be responsible for developing and implementing complex case plans that aim to achieve desired outcomes in the context of the child or young person's ongoing safety, stability and developmental needs. This will involve working closely with families to support the development and implementation of these plans.
Key Responsibilities:
- CASE PLAN DEVELOPMENT: Develop and implement case plans that are tailored to the specific needs of each family.
- FAMILY SUPPORT: Provide ongoing support to families through regular home visits, phone calls and other forms of communication.
- RISK ASSESSMENT: Conduct thorough risk assessments to identify potential risks and develop strategies to mitigate them.
Requirements:
- QUALIFICATIONS: Bachelor degree in social work or a related field.
- EXPERIENCE: Demonstrated experience in case management, preferably with children, young people and families with complex needs.
- SKILLS: Strong interpersonal and communication skills, as well as the ability to work effectively under pressure.
