Working collaboratively within a multidisciplinary team, these professionals develop and deliver evidence-based support plans that foster participant autonomy and independence.
Key Responsibilities:- Conduct comprehensive assessments and person-centred planning to identify participant needs.
- Develop tailored support plans that promote participant development and overall wellbeing.
- Collaborate with teams to establish meaningful goals and practical strategies that reflect participant strengths and passions.
- Enhance the knowledge, capacity, and strengths of those who support participants in their lives.
- Provide guidance and mentorship to less experienced Behaviour Support Practitioners to ensure high-quality practice.
