Job Description

As a Clinical Care Coordinator, you will be a vital part of our multidisciplinary team delivering care coordination services for children and youth. You will contribute to high-quality evidence-based clinical care by providing support and guidance to young people up to the age of 25. Your role will also involve addressing service gaps by enhancing accessibility and integrating primary mental health care, psychosocial support, and early intervention.

Required Skills and Qualifications
  • AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist, or AASW registration/eligibility as a Social Worker, or Level 2 ACA registration/eligibility
  • Knowledge and understanding of evidence-based interventions and clinical practice in the areas of mental health and alcohol and drug use
  • Excellent communication (including written) and interpersonal skills
  • Ability to engage with people, advocate on their behalf, and establish and maintain effective relationships with staff, clients, families & friends and stakeholders
  • Experience in electronic client/customer management systems, desktop computer, and Microsoft Office applications
