Community Health Specialist

We are seeking a skilled and compassionate Registered Nurse to join our team in the Ballarat area.
The ideal candidate will have experience in community nursing, with a strong focus on providing high-quality patient care and support.
The successful applicant will be required to provide clinical assessments, reviews and care needs for clients in their homes, as well as offer clinical advice and support where needed.
This is a permanent full-time opportunity, working Monday to Friday during business hours. Flexibility is negotiable, depending on the needs of the organisation and clients.
About You
- Bachelor of Nursing or equivalent qualification with current AHPRA Div 1 Nursing registration (required).
- Minimum 3-5 years experience as a Registered Nurse in Community, Commonwealth Home Support or District Nursing (required).
- Team player, flexible in your approach to work with a positive outlook.
- Endorsed for Continence Assessments and Prescription (highly desirable).
- Experience in actively visiting clients in their homes.
- Experience with evaluating and implementing care support plans.
- Familiarity with the My Aged Care portal.
- Proficiency in decision making, critical analysis and on the spot problem solving.
- Demonstrated abilities to delegate and prioritise in a fast paced environment with exemplary communication skills in a team and one on one interactions.
- Availability to work Full time, Monday to Friday 830am - 430pm.
- Own your own registered and insured vehicle.
- Thrive in a team and also appreciate the one to one interactions.
- Reliable, team spirited and devoted to person centered care.

Requirements
- A current Working With Children Check card.
- Current and ongoing flu vaccinations.
- Australian working rights.
- Ability to pass a national police clearance and NDIS Clearance.
- Full driver's licence and comprehensive car insurance and car registration in your name.
