- This role involves developing customer care plans and annual budgets for Home Care Package Customers;
- Conducting skills assessments, providing training, coaching and guidance to our team of Community Care Workers;
- Review and monitor medications, complex clinical care including palliative care, bowel care/suppositories, catheter care, stoma care, oxygen and more;
- Monitoring and communicating on the customer's wellbeing and reporting on progress of customers;
- Develop strategies which will address the identified needs of the customer in consultation with their representative/s where appropriate in accordance with their funding guidelines;
- Effecting management of the care needs of customers and demonstrate effecting utilisation of available resources;
To be successful in this role, you will need:
- A Qualified Enrolled/Registered Nurse with a minimum of 3 years experience;
- Proven ability to provide technical and practical medical procedural training;
- Proven medical experience; Particularly: Enteral Feeding - Epilepsy Management - Infection Control - Tracheostomy Care - Catheter and Bowel care;
- Experience working with children and/or young people with disabilities;
- Senior first aid and CPR qualification;
- Strong verbal and written communication skills;
- A legal right to work in Australia;
- Current AHPRA Registration;
- Australian drivers licence (minimum P2);
- Recent Police Clearance;
- First Aid Certificate;
- CPR;
- NDIS Worker Screening Check;
- NDIS Worker Orientation Module;
- Working With Children Check for employees;
- Manual handling training;
- Behavioural support training;
- Up to date with your COVID19 vaccinations;
