  • This role involves developing customer care plans and annual budgets for Home Care Package Customers;
  • Conducting skills assessments, providing training, coaching and guidance to our team of Community Care Workers;
  • Review and monitor medications, complex clinical care including palliative care, bowel care/suppositories, catheter care, stoma care, oxygen and more;
  • Monitoring and communicating on the customer's wellbeing and reporting on progress of customers;
  • Develop strategies which will address the identified needs of the customer in consultation with their representative/s where appropriate in accordance with their funding guidelines;
  • Effecting management of the care needs of customers and demonstrate effecting utilisation of available resources;
Required Skills and Qualifications

To be successful in this role, you will need:

  • A Qualified Enrolled/Registered Nurse with a minimum of 3 years experience;
  • Proven ability to provide technical and practical medical procedural training;
  • Proven medical experience; Particularly: Enteral Feeding - Epilepsy Management - Infection Control - Tracheostomy Care - Catheter and Bowel care;
  • Experience working with children and/or young people with disabilities;
  • Senior first aid and CPR qualification;
  • Strong verbal and written communication skills;
  • A legal right to work in Australia;
  • Current AHPRA Registration;
  • Australian drivers licence (minimum P2);
  • Recent Police Clearance;
  • First Aid Certificate;
  • CPR;
  • NDIS Worker Screening Check;
  • NDIS Worker Orientation Module;
  • Working With Children Check for employees;
  • Manual handling training;
  • Behavioural support training;
  • Up to date with your COVID19 vaccinations;
