We are seeking a highly skilled Nurse Practitioner to join our Respite Care team in Butler, Western Australia.
About the RoleAs a Nurse Practitioner, you will be responsible for delivering high-quality patient care and support, ensuring the physical and emotional needs of residents are met.
- Develop and implement individualised care plans, taking into account the unique needs and preferences of each resident.
- Collaborate with healthcare professionals to provide comprehensive care and support, ensuring seamless transitions and outcomes.
- Provide education and support to residents and their families, empowering them to make informed decisions about their care.
- Maintain accurate records and reports, tracking resident progress and identifying areas for improvement.
- Participate in quality improvement initiatives, contributing to the development of best practices and protocols.
- A current Australian 'C' Class Driver's Licence with no restrictions.
- AHPRA registration as a Nurse Practitioner.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and healthcare professionals.
