Job Summary:

We are seeking a skilled and compassionate Client Care Coordinator to join our team.

Key Responsibilities:

  • Coordinating Client Care Needs:
  • A key aspect of this role is to assess, develop and implement individualized care plans for clients in conjunction with the clinical management team.
  • Providing Support and Supervision:
  • This includes providing guidance and support to care assistants to ensure high-quality care delivery.
  • Liaising with Clients and Teams:
  • The successful candidate will also be responsible for liaising with clients, their families, and members of the multidisciplinary team to achieve positive client outcomes.

Requirements:

  • Clinical Knowledge and Experience:
  • The ideal candidate will possess current registration with the Australian Health Practitioner Regulation Agency (AHPRA) and relevant clinical experience.
  • Certifications and Skills:
  • A current CPR Certificate and strong communication skills are essential for this role.
  • Roster Flexibility:
  • The ability to work a varied roster including night and weekend shifts is also required.
