Job Overview

The Healthcare Navigator for Vulnerable Youth is a critical role in supporting the well-being of children and young people in Out of Home Care. This position requires an individual with advanced knowledge of the healthcare system and its responses to the needs of vulnerable youth.

Main Responsibilities
  • To oversee the development of health assessments and plans for children and young people in Out of Home Care, working closely with multidisciplinary teams.
  • To lead a small team of clinicians in providing high-quality care and support to vulnerable youth, ensuring that their physical, emotional, and psychological needs are met.

This role demands strong leadership skills, the ability to take innovative approaches to complex situations, and excellent program management experience with multiple stakeholders.

The successful candidate will be expected to have a deep understanding of child protection policy and practice, as well as knowledge of associated health pathways.
