We are seeking a skilled Clinical Care Specialist to join our team. This role involves working collaboratively with clients, care managers, and other healthcare professionals to deliver high-quality clinical care and support to individuals in their homes.
Key responsibilities include:
- Providing comprehensive nursing care to clients, including assessments, planning, implementation, and evaluation of client-centered care plans;

Required Skills and Qualifications:- Degree in Nursing (Registered Nurse);
