We are seeking a skilled Care Manager to join our multidisciplinary team in a Full-time capacity.
The successful candidate will be responsible for providing quality case management services, supporting home care package consumers and their carers, and developing and documenting consumer-directed care plans.
Key Responsibilities:
- Providing support and advice to home care package consumers and their carers to help them achieve their goals and remain living at home and in their community.
- Developing and documenting consumer-directed care plans and assessments with regard to their needs and preferences.
- Providing ongoing care management for Home Care Packages, conducting regular reviews of care needs and goals, and amending care plans and supports as necessary.
- Liaising and advocating with other relevant service providers regarding individual care needs.
Requirements:
- Minimum of 3 years' experience in Home Care Packages program and/or Tertiary qualification in Social Work, Allied Health, Nursing, Care Management, or equivalent.
- Extensive experience working with older people and/or with Home Care Packages consumers.
- Demonstrated experience in Consumer Directed Care (CDC) choices and preferences to consumers.
- Knowledge of aged care related specialist services, i.e., Parkinson's and Dementia support Services would be an advantage.
- Knowledge of budget management and fees and costs associated.
