Client Support Specialist

We are seeking dedicated individuals to join our Community Aged Care team as Client Support Specialists.

Key Responsibilities
  • Deliver exceptional Home Care services through assessment, care planning, case-management and coordination of care for Self-Managed or Level 1-2 Home Care Package customers.
  • Support customers who are well and mostly independent with intermittent decline, ensuring ongoing assessment, review, care planning, monitoring and advocacy in line with organisational procedures.
  • Assist customers in navigating the aged care system, guaranteeing they receive a comprehensive range of options to meet their needs.
  • Manage Home Care Package Budgets for customers effectively, ensuring they receive the necessary services within their allocated funding.
Requirements
  • Proven care or service experience, preferably in a Community Aged Care setting.
  • Ability to identify customer care and service needs and respond promptly to care escalations.
  • Understanding of the needs of older individuals living in the community.
  • Excellent communication skills, emotional intelligence, and ability to build rapport with customers and stakeholders quickly.
  • Proven customer service skills, administrative and organisational skills, and ability to produce accurate and timely documentation.
