The Family Support Specialist will work collaboratively with families to empower and build their capacity for positive change. This role involves engaging families in their homes, translating initial engagement into a strong ongoing partnership.
Key Responsibilities- Meet performance targets as per the weekly, monthly, and quarterly dashboard system.
- Deliver high-quality practice in all interactions with clients, consistent with our values and vision.
- Provide effective case management practices and record keeping that meet operational requirements.
- Foster positive engagement through an assertive outreach approach that engages clients into the service.
- Facilitate client-led assessments and care plans to identify client goals, including the entire family where appropriate.
- Provide targeted interventions and family-facilitated group meetings to achieve client goals.
- Commit to a high level of quality management through the use of quality management systems and tasks.
