Aged Care Pharmacist Role

This role presents an exceptional opportunity to make a tangible difference in the lives of aged care residents. As an Aged Care On-site Pharmacist, you will be integral to partnered residential aged care facilities.

Key Responsibilities:
  • Conduct regular medication reviews and reconciliation, particularly during transitions of care, ensuring safe and effective management of medications.
  • Collaborate with General Practitioners, nurses, and facility staff to resolve medication-related issues, fostering a multidisciplinary approach to resident care.
  • Lead and participate in case conferences and GP rounds, driving collaborative efforts to improve resident outcomes.
  • Provide expert guidance to the Medication Advisory Committee, assisting in establishing MACs where needed, and implementing quality use of medicines initiatives.
  • Develop and implement deprescribing strategies, audits, and other quality improvement initiatives to optimize resident care.
  • Deliver culturally sensitive and person-centered care, respecting resident preferences and values.
  • Educate facility staff on medication protocols, high-risk medications, and regulatory compliance, promoting a culture of excellence in aged care.
  • Maintain accurate records and contribute to continuous improvement processes, ensuring seamless delivery of care.
Requirements:
  • Pharmacy Registration: Current registration with the Pharmacy Board of Australia is mandatory.
  • Credentialing: Completion of ACOP Tier 1 training modules (via approved provider) is essential.
  • Vaccination Status: Up-to-date vaccination status is required, including RACF-recommended vaccinations.
  • Police Check: A national police clearance suitable for aged care work is necessary.
  • Professional Indemnity Insurance: Valid and current coverage is essential.
Desirable Qualifications:
  • Minimum 2 years' experience in community pharmacy or clinical pharmacy setting.
  • Prior exposure to aged care, medication reviews, or HMRs is advantageous.
  • Vaccinating pharmacist ideal.
  • Strong understanding of psychotropics, antimicrobials, and high-risk medications.
  • Excellent communication and stakeholder engagement skills.
  • Ability to work autonomously and collaboratively across multidisciplinary teams.
  • Familiarity with clinical governance, quality use of medicines principles, and aged care compliance frameworks.
