This is a permanent part-time opportunity for registered nurses to work with acute patients in their homes, providing expert care and support to those who need it most.

Your Key Responsibilities:

  • Administer antineoplastic drugs to patients in their homes, working independently and collaboratively with the medical team and ward staff to ensure smooth care transfer.
  • Work with a variety of acute patients, including those requiring chemotherapy and hospital-level care at home, using your advanced clinical assessment and problem-solving skills to provide effective care.
  • Collaborate with the medical team and ward staff to ensure seamless communication and coordination of care.

Required Skills and Qualifications:

  • Current nursing registration with AHPRA
  • Valid Full Australian Driver's License (a driving assessment will be conducted)
  • Understanding of community-based services and the ability to navigate complex healthcare systems.
  • Demonstrated autonomous practice and strong prioritisation, time management, and organisational skills.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
  • Advanced clinical assessment and problem-solving skills, with the ability to escalate clinical deterioration when necessary.
  • Commitment to ongoing education and professional development, with a focus on staying up-to-date with the latest clinical practices and guidelines.
  • Proficiency in Microsoft Office, with experience using electronic health records and other healthcare software.

Desirable Skills:

  • Experience in both Hospital in the Home (HITH) and Chemotherapy in the Home (CITH), with knowledge of PICC and CVAD management.
  • Complex wound care expertise, with current IV cannulation certification and venepuncture experience.
  • Management of IDC and SPC, drain tubes, with a focus on delivering high-quality patient care in challenging environments.
