Job Title: Model of Care Coach

Location: Macquarie Park, NSW

About the Role:

We are seeking a highly skilled and experienced professional to join our team as a Model of Care Coach. As a key member of our team, you will play a critical role in driving change and improvement in our Residential portfolio.

Key Responsibilities:

  • Partner with business leaders and their teams to co-design, implement, and execute the new Inspirit Model of Care.
  • Work with internal and external stakeholders to co-design the new model of care at an operational level.
  • Draw on your experience in aged care to redesign processes, test, and apply action learning to results.
  • Travel to sites across Sydney Metro and Regional NSW, with extended stays of up to approximately 3 weeks.

Requirements:

  • Completion of tertiary qualifications in Project Management, Lean/Human Centred Design/Change and Business or Service Delivery, or significant project management experience in Change and Continuous Improvement.
  • Minimum 5 years' demonstrated experience in RAC or HCS business stream at an operational level.
  • Strong analytical ability, problem-solving skills, and big picture mindset.
  • Strong facilitation and workshop skills, ability to provide constructive feedback.
  • Proficient MS Office and MS Project skills.
