We are seeking a compassionate and skilled professional to provide personalized support and guidance to individuals living with mild to moderate dementia and their family carers.

The ideal candidate will possess a deep understanding of the needs of aged care clients and their families, as well as excellent communication and interpersonal skills.

They will be responsible for delivering individualized support, making referrals when necessary, and facilitating peer support groups to promote a sense of community and belonging.

This role requires a high level of empathy, active listening, and problem-solving skills, with the ability to work effectively in a team environment and prioritize tasks efficiently.

The successful candidate will have a strong commitment to empowering individuals to live their best life, and will be passionate about delivering high-quality services that make a meaningful impact on the lives of those we serve.

Key responsibilities will include:

  • Providing counseling and emotional support to individuals living with dementia and their family carers
  • Delivering individualized support plans tailored to meet the unique needs of each client
  • Making referrals to external agencies or services as required
  • Facilitating peer support groups to promote social connections and a sense of community
