Transition Support Professional

About the Role:

We are seeking a Transition Support Professional to join our team. The successful candidate will provide specialist transition support services to young people who are at risk of homelessness, based on the principles of the Strengthening Families Protecting Children Framework for Practice and the core elements of the Aboriginal and Torres Strait Islander Child Placement Principle.

About the Job:

  • Key Responsibilities:
  • Provide specialist transition support that identifies and responds to the complex needs of high-risk young people, promoting their connection to family, culture, and community.
  • Promote and facilitate involvement of young people in goal setting and planning for their transition, ensuring they have the skills and knowledge to maintain safe and stable housing and improve their wellbeing.
  • Organise provision of specialist services addressing high-risk and complex needs of young people, collaborating with government and non-government agencies to ensure efficient service delivery.
  • Cultivate relationships with regional line managers and senior program officers to enhance quality improvement, align with regional priorities, and identify opportunities for outcome improvement.
  • Maintain sound record keeping relevant to service delivery, contributing to programmatic data reporting systems.

Requirements:

  • Highly developed specialist transition practice knowledge and skills.
  • Strong interpersonal and communication skills, with ability to build effective relationships.
  • Ability to work collaboratively with various stakeholders, including government and non-government agencies.
  • Demonstrated understanding of child safety and external agencies.
