Key Responsibilities
  • Conduct Comprehensive Assessments: Conduct thorough assessments of residents' physical, emotional, and social needs to identify areas for improvement.
  • Develop Individualised Care Plans: Collaborate with residents, families, and healthcare professionals to develop tailored care plans that meet the unique needs of each resident.
  • Coordinate Care Services: Oversee the implementation of care plans to ensure they are consistently followed and adjusted as necessary.
  • Monitor Resident Health and Well-being: Regularly monitor residents' health and well-being, intervening promptly to address any changes or concerns.
  • Facilitate Communication: Act as a liaison between residents, families, staff, and external healthcare providers to facilitate seamless communication and continuity of care.
  • Manage Staffing and Performance: Manage staffing levels, rostering, and performance to ensure adequate and skilled care provision.
  • Ensure Compliance: Ensure compliance with regulatory standards and organisational policies.
  • Foster a Supportive Environment: Foster a supportive and respectful environment that promotes the dignity, independence, and safety of residents.
Requirements
  • Qualifications: Bachelor's degree in nursing, or a related field (preferred).
  • Experience: Proven experience in a supervisory or management role within aged care or a similar healthcare setting.
  • Clinical Skills: Strong clinical assessment and care planning skills.
  • Communication Skills: Excellent communication, interpersonal, and organisational abilities.
  • Knowledge: Knowledge of aged care regulations and standards.
