Care Coordinator for Maternal and Child Health

Job Description:
We are seeking a dedicated Care Coordinator to join our team in Alice Springs. In this rewarding role, you will work closely with eligible women from early pregnancy to support them in recognizing their strengths, building skills and confidence, accessing services, and improving their life course.
Key Responsibilities:
- Develop long-term relationships with clients based in Alice Springs to support them in achieving their goals and improving their life course.
- Work collaboratively with healthcare professionals to deliver evidence-based care programs.
- Promote autonomy, flexibility, and personal satisfaction in your work with vulnerable populations.
Requirements:
- A strong understanding of maternal and child health principles.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
