**Job Opportunity: Mobile Case Manager**

We are seeking a skilled and compassionate Clinical Outreach Specialist to join our team in delivering personalized face-to-face assistance to workers and employers.

About the Role
  • This full-time 12-month contract position requires you to be on the road servicing your appointed area 4 days per week and working from home 1 day.

As a Clinical Outreach Specialist, you will play a vital role in helping people get their lives back through ongoing support during their return-to-work journey.

Your Responsibilities
  1. Deliver timely and personalized face-to-face assistance to workers and employers over a defined period of time as agreed with the Case Manager.
  2. Provide expert advice to stakeholders including workers, employers and providers.
  3. Conduct community visits inclusive of but not limited to work sites or medical clinics.
  4. Proactively achieve the optimum outcome of recovery and early sustainable return to work for workers.
  5. Consult with the Case Manager, Providers, Employers and Return to Work Specialist where required.
About You
  • You will have proven case management experience and ability to return people to the workforce.
  • You should have knowledge of the NSW workers compensation legislation and regulations.
  • You must be an effective listener and conflict resolution skills, with the ability to understand the needs of people recovering from a physical or psychological injury.
  • You will require excellent attention to detail, organisational skills, written and verbal proficiency.
  • You will need understanding of medical terminology and injury recovery timeframes.
  • You must own a vehicle with a 5-star ANCAP rating, insurance & current unrestricted NSW Driver's License.
