After Hours Care Provider

As a Hospice Nurse, you will provide care and support to patients and families in their homes. You will work autonomously to assess the physical, psychosocial, and spiritual needs of your patients and develop individualized care plans.
Responsibilities:- Conduct thorough assessments of patient needs and develop comprehensive care plans.
- Provide skilled nursing services, including wound care, medication management, and vital sign monitoring.
- Collaborate with patients' physicians to ensure high-quality care and make recommendations for improvement.
- Communicate effectively with patients, families, and healthcare teams to coordinate care and address concerns.
- Graduate of an accredited school of nursing with a Bachelor's degree preferred.
- Licensed RN/LPN in the state of Ohio.
- Experience in pain and symptom management, oncology, or mental health preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively as part of a team.
