After Hours Nurse

This is a unique opportunity to make a positive impact in the lives of others as an after-hours nurse. You will have the autonomy to utilize your nursing skills and provide care to patients and families who need help at night.
As an after-hours nurse, you will be relied upon to take care of patients and families who are facing uncertain times. You will have the honor of becoming a trusted face and making a significant difference in their experiences.
Responsibilities- A Assess physical, psychosocial, and spiritual needs of the patient and family (RN only).
- G Gather data related to patient and family's physical, psychosocial, and spiritual needs (LPN).
- P Provide intermittent skilled nursing services to the patient under the patient's physician's direction.
- M Monitor and evaluate pain and symptoms; provide information and assistance to the patient and family to manage those problems.
- S Serve as a liaison to the patient's physician(s) when needed; provide information to the physician based on knowledge relating to the patient's needs.
- P Provide emotional support to the patient and family.
- R Refer patients and/or family members to other services when appropriate.
- P Provide instruction (healthcare teaching) to the patient and family as related to care in a residence.
- M Make visit to patient's residence at the time of death.
- M Maintain, promptly complete, and return all documentation as required.
- C Communicates and acts as liaison with patients, family, and healthcare professionals in a manner that respects confidentiality and dignity.
- R Relay pertinent information to appropriate team members.
- P Participate in meetings as initiated by team leader.
- P Participate in mandatory or elective events, trainings, or in-services as necessary.
- RN's shall participate in triage coverage as needed.
