**Job Description:
- Provide routine personal care services to individuals in various healthcare settings or in their homes.
- Support participants with physical, developmental, or cognitive disabilities to maintain their independence and improve their overall quality of life.
- Assist with daily activities, such as bathing, dressing, and eating, as well as provide emotional and social support.
- Monitor the safety and well-being of participants and report any concerns to the Care Service Manager.
- Participate in planning the care of individuals and follow therapy plans to assist those with dementia and behavioral problems.
- Assist with rehabilitation exercises, basic treatment, and deliver medications as directed.
- Work with participants, family members, and other professionals to support individual goals.
- Develop, implement, and evaluate training and behavior programs to attain identified individual goals.
- Ensure accurate and timely documentation of support provided.
**Responsibilities and Duties:
- Supervise and assist with showering, bathing, and toileting.
- Assist with rising and retiring, dressing, and undressing.
- Provide assistance with dental care, hair washing, grooming, and shaving.
- Assist with mobility, including transferring to a commode or wheelchair.
- Monitor well-being and report any concerns to the Care Service Manager.
**Work Health and Safety Responsibilities and Duties:
- Participate in staff consultation processes around the Disability Work Health and Safety Policy.
- Undertake compulsory WHS training.
- Ensure all hazards are reported and addressed promptly.
- Report all incidents, accidents, and injuries promptly.
- Carry out duties with diligence and due care for personal safety and the safety of others.
**Requirements:
- Hold a relevant AQF Certificate III or equivalent qualification.
- Have 12 months of relevant work experience or part-time equivalence in lieu of the qualification.
- Be fluent in Polish language due to the needs of aged care residents/clients.
- Hold a current AFP check, driving license, and statutory declaration re no criminal records in other countries of residence.
