We are seeking a compassionate and skilled Support Practitioner to join our Children, Youth, and Families team in Ashmore & Daisy Hill.

  • Facilitate intake, screening, and placement of children and young people with foster and kinship care families.

  • Develop and evaluate care plans tailored to individual needs.

  • Lead case management planning and maintain accurate client records.
  • Provide flexible, in-home support services, including after-hours and on-call duties.

As a Support Practitioner, you will play a vital role in providing high-quality support to children, young people, and families, ensuring their safety and well-being.

Required Skills and Qualifications
