Comprehensive Community Healthcare Professional

Our innovative service model focuses on delivering the best clinical outcomes for clients. As a District Nurse, you will be responsible for the delivery of comprehensive services across home care clients in the Melbourne area.
Duties:- Conduct comprehensive clinical assessments and develop clinically appropriate care plans.
- Provide a range of nursing services to clients within the community, including but not limited to medication management, wound care, diabetes management, catheter care, stomal therapy, and cognitive assessments.
- Work holistically with other healthcare professionals to provide a multidisciplinary approach to client care.
- Educate clients and their families to optimise the client's ability to live safely within the community.
- Work collaboratively with case managers and NDIS support coordinators to provide positive client outcomes.
- Work autonomously to prioritise and complete tasks and complete all required documentation to a quality standard.
- Unrestricted registration with AHPRA as a registered nurse.
- Availability Tuesdays and Thursdays.
- Sound clinical knowledge and skills.
- Experience working with clients who have chronic and complex health needs.
- Experience working with clients from diverse cultural and socio-economic backgrounds.
- Excellent verbal and written communication skills.
- Excellent time-management skills and attention to detail.
- National police check clearance.
- Working with children check and NDIS worker screening.
- Current CPR certificate.
