This is an exciting opportunity to join our team as a Clinical Nurse Specialist Grade 2. In this role, you will have the opportunity to lead, inspire, and make a real difference in the lives of patients as they move from hospital to home.

Key Responsibilities:
  • Lead and inspire a dedicated team, fostering a culture of collaboration, excellence, and professional growth.
  • Ensure safe, patient-centred care for individuals requiring acute and post-acute services, including Hospital in the Home.
  • Work collaboratively with multidisciplinary teams to ensure smooth transitions and timely access to care across the healthcare journey.
  • Champion evidence-based practice, quality improvement, and service development initiatives to enhance patient outcomes.
  • Apply advanced clinical expertise to provide guidance, education, and support to staff, patients, families, and carers.
About You:
  • Registered Nurse (Division 1) with current AHPRA registration and relevant postgraduate qualifications.
  • At least 3 years' clinical experience in acute, post-acute, or Hospital in the Home care.
  • Confident applying advanced nursing skills in complex situations.
  • Passionate about providing safe, patient-centred care, especially for individuals transitioning from hospital to home.
  • Strong clinical leadership, mentoring and supporting other nurses while promoting professional growth.
