Responsibilities

The Home Health RN Patient Care Manager supervises and coordinates clinical services, providing and directing nursing care to patients in their homes as prescribed by the physician, in compliance with applicable laws, regulations and agency policies.

  1. Provide clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
  2. Receive referrals, assign clinicians appropriately, evaluate patient orders, and initiate start of care visits.
  3. Coordinate determination of patient home health benefits, medical necessity, and insurance approvals.
  4. Assess patient needs continually and ensure care is individualized, appropriate, and meets eligibility criteria and physician orders.
  5. Review assessments and care plans daily, providing recommendations to clinicians as appropriate.
Education and Experience
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and access to reliable transportation.
