Clinical Care Coordinator and Nurse Mentor Role

We are seeking a highly skilled Clinical Care Coordinator and Nurse Mentor to join our team in Knoxfield, Victoria. The successful candidate will be responsible for managing and resourcing the care needs within the facility, ensuring that all residents receive the highest quality of clinical care. This is an excellent opportunity for a registered nurse looking to specialise in palliative, dementia, IPC, falls and/or wound care.

The ideal candidate will have a minimum of 1 year experience as a Care Coordinator or similar leadership position and hold a current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA). Post graduate qualifications in aged care would be advantageous but not essential. The successful candidate will also have extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC and accreditation compliance and documentation.

Key Responsibilities
  • Provide clinical expertise and leadership to the clinical and care service teams across the Care Home.
  • Assist the Clinical Care Manager to lead carer teams to meet all clinical needs of residents – including monitoring and tracking clinical indicators to achieve best practice clinical care outcomes.
  • Manage all aspects of the assessment, documentation and submission of AN-ACC requirements within the facility.
  • Support the effective documentation and full clinical team user support of policies, procedures, and IT system requirements in the facility.
  • Coordinate assessment, care planning and review processes within the facility.
  • Support the admission of new residents to facilities for optimal care assessments and initial consultations with resident and families.
  • Ensure accurate information is recorded and in accordance with all organisational and legislative requirements (AN-ACC, resident records and staff time recording).
  • Input and oversight into the facility team rosters and ensuring optimal skill mix available for delivery of care to residents.
  • Manage and document clinical risk within the facility and ensure a commitment to continuous improvements within the facility.
