Community Health Transport Specialist

We are seeking a dedicated Transport Coordinator to join our team. As a key member of our social health team, you will support our community by safely transporting clients to medical appointments and back home.
Main Responsibilities- Transportation Management: Safely transport patients to and from medical appointments as per scheduled routes.
- Vehicle Maintenance: Ensure the day-to-day cleanliness and safety of your assigned vehicle.
- Route Efficiency: Optimize patient pick-ups and drop-offs according to established transport schedules.
- Booking System: Complete bookings for transports in our designated system.
- Patient Pick-up: Collect clients on time at their designated addresses.
- Stakeholder Relationships: Foster positive relationships with healthcare organizations, hospitals, and other stakeholders.
- Culture Development: Contribute to a culture of continuous improvement within the team.
