Senior Rehabilitation Therapist
This role offers a unique combination of Occupational Therapy and Care Coordination, providing an exciting opportunity to make a meaningful impact on clients' lives.
- Occupational Therapy Services: As a highly skilled Occupational Therapist, you will deliver comprehensive assessment, intervention, and management services to help clients achieve optimal functional recovery, promote empowerment, and improve their overall wellbeing.
- Care Coordination:As a skilled Care Coordinator, you will provide person-centred care coordination for high-risk clients with complex aged care needs, comorbidities, and health issues. You will conduct holistic assessments, coordinate all aspects of healthcare and social care, and facilitate interventions to optimise clients' safety, wellbeing, and self-management skills.
The role involves working with clients in the community who experience various challenges impacting their functional independence, safety, and occupational performance. You will work collaboratively with clients, families, and healthcare professionals to develop effective strategies and implement interventions that promote client autonomy and quality of life.
You will be responsible for conducting home assessments, prescribing equipment, developing falls prevention plans, managing pressure care, and retraining clients in activities of daily living. Additionally, you will monitor client progress, facilitate goal achievement, and ensure seamless linkages with hospital and community services.
This challenging yet rewarding role requires strong communication, interpersonal, and organisational skills, as well as the ability to work effectively in a dynamic environment.
Successful candidates will possess a bachelor's degree in Occupational Therapy or a related field and relevant postgraduate qualifications or certifications. They will also have extensive experience in Occupational Therapy, preferably in complex care settings, and demonstrate excellent problem-solving, time management, and adaptability skills.
