Care Coordination Specialist

As a key player in our multidisciplinary care teams, your role is to provide exceptional person-centred care to our clients by overseeing and coordinating complex clinical needs through comprehensive assessments, strategic planning, and seamless service coordination.

Main Responsibilities:
  • Develop, review, and implement individualized care plans in collaboration with the client and healthcare professionals.
  • Manage budgets and facilitate effective care activities.
  • Liaise with healthcare teams to monitor and respond to evolving client needs.
  • Continuously assess, review, and evaluate care plans, services, and outcomes.

You will primarily work within the community, engaging with clients on a daily basis.

Necessary Qualifications and Skills:
  • A registered nurse or allied health professional with current AHPRA registration and unlimited work entitlements.
  • Demonstrated expertise in client-centred, community-based care services delivery.
  • Proven experience and knowledge of Community Funding streams and programs.
  • Excellent communication and clinical problem-solving skills.
  • A valid Australian driver's licence and a roadworthy, insured vehicle for work purposes.
