Job Summary

We are seeking an experienced Registered Nurse to join our Community Palliative Care team as a Clinical Support Role. This fixed-term, part-time position is ideal for a dedicated and caring professional who wants to make a real difference in the lives of patients and their families.

In this role, you will work collaboratively with our multidisciplinary team to deliver high-quality, holistic care to patients in their homes and community settings.

You will have the opportunity to develop your skills and expertise in palliative care, working closely with patients, families and carers to identify and address their physical, emotional and spiritual needs.

This is a fantastic opportunity for a Registered Nurse looking to specialise in gerontology or palliative care, or those who want to build on their existing experience and skills.

Key Responsibilities
  1. Provide high-quality, person-centred care to patients in their homes and community settings
  2. Work collaboratively with the multidisciplinary team to assess and address the physical, emotional and spiritual needs of patients and their families
  3. Develop and maintain effective relationships with patients, families and carers to ensure seamless care transitions and coordination
  4. Participate in quality improvement initiatives to enhance patient outcomes and experiences
Required Skills and Qualifications

To be successful in this role, you will need:

  • A current Victorian registration as a Registered Nurse
  • Experience in palliative care or gerontology (or willingness to develop these skills)
  • Strong communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Flexibility to adapt to changing priorities and demands
