Role Overview
We are seeking a skilled and compassionate Home Health RN Patient Care Manager to join our team. As a key member of our home health team, you will oversee the delivery of high-quality patient care services in patients' homes, working closely with patients, families, and healthcare professionals.
Key Responsibilities
- Supervise and coordinate clinical services, providing and directing nursing care to patients in their homes as prescribed by the physician.
- Continuously assess patient needs, ensuring care is individualized, appropriate, and meets eligibility criteria and physician orders.
- Coordinate determination of patient home health benefits, medical necessity, and insurance approvals.
- Review assessments and care plans daily, providing recommendations to clinicians as appropriate.
Requirements
- Current RN licensure in Australia
- Valid driver's license and access to reliable transportation
