The ideal candidate will possess:
- A current AHPRA Nursing registration.
- A valid driver's license and access to a reliable vehicle.
- Excellent verbal and written communication skills.
- High-level organizational skills, ensuring efficient management of client care plans and records.
- Proficient computer skills in Microsoft Office and Excel (or similar software).
As a Clinical Care Coordinator, your primary responsibilities will include:
- Overseeing and implementing quality improvement initiatives to ensure exceptional client care services.
- Maintaining accurate, up-to-date client care plans, records, and meeting administrative requirements according to policies, procedures, and funding guidelines.
