About the Role

Deliver client-centric support that empowers individuals on their recovery journey.

Working within a recovery-oriented and trauma-informed framework, you will provide culturally responsive case management in line with established frameworks.

Key responsibilities include conducting client intake assessments, developing strengths-based case plans, and delivering ongoing outreach support.

Key Responsibilities:
  • Provide case management services in accordance with best practice guidelines
  • Develop and implement client-centered case plans to achieve goals
  • Support clients with day-to-day living skills, medication observation, and service navigation
  • Monitor and review client progress towards goals
  • Contribute to team deliverables under direction
  • Build effective working relationships with internal teams, stakeholders, and community partners
  • Complete administrative duties ensuring accurate data entry and reporting

Requirements for Success:

  • Relevant tertiary qualification or equivalent experience in the social work and/or mental health space
  • Experience in case management or support work within related services
  • Knowledge of recovery-oriented practice and ability to work collaboratively with clients to achieve their goals
  • Strong interpersonal and communication skills; able to build trust and rapport
  • Ability to work autonomously under general supervision and manage competing priorities
  • Committed to values-driven practice
  • Valid driver's license required
