Key Responsibilities
  • Lead inter and multidisciplinary care planning, acting as an information conduit within the organisation and with external partners.
  • Enhance knowledge, skills, and confidence in working with neurodiversity and mental illness, and understanding NDIS and Early Childhood Early Interventions (ECEI) processes via multi-disciplinary and multi-agency teams.
  • Collaborate with families to create and implement care plans, complete referrals, and liaise with schools and agencies.
  • Assist in developing systems, processes, and tools for consistent engagement, referral, and transition, including managing an allied health service map.
  • Coordinate MDT meetings, collate client information, and align goals between families and MDT.
Requirements

Champion for Care Planning: Lead inter and multidisciplinary care planning, acting as an information conduit within the organisation and with external partners.

Systems Improvement: Enhance knowledge, skills, and confidence in working with neurodiversity and mental illness, and understanding NDIS and Early Childhood Early Interventions (ECEI) processes via multi-disciplinary and multi-agency teams.

Key Contact for CAMHS: Collaborate with families to create and implement care plans, complete referrals, and liaise with schools and agencies.

Development of Systems and Tools: Assist in developing systems, processes, and tools for consistent engagement, referral, and transition, including managing an allied health service map.

MDT Facilitation: Coordinate MDT meetings, collate client information, and align goals between families and MDT.

Project Work: Develop information tools, consult on collaborative information portals, and create individualized referral pathways for neurodiverse individuals.

Consultation-Liaison Role: Provide support and information on best practices and appropriate services for neurodevelopmental and mental health needs.

About This Role

This is a champion role for care planning, requiring excellent communication and interpersonal skills. The successful candidate will have experience in working with neurodiversity and mental illness, and understanding NDIS and Early Childhood Early Interventions (ECEI) processes.

The ideal candidate will be able to lead inter and multidisciplinary care planning, enhance knowledge, skills, and confidence in working with neurodiversity and mental illness, and collaborate with families to create and implement care plans.

Additionally, they will be able to assist in developing systems, processes, and tools for consistent engagement, referral, and transition, and coordinate MDT meetings, collate client information, and align goals between families and MDT.

They will have experience in project work, developing information tools, consulting on collaborative information portals, and creating individualized referral pathways for neurodiverse individuals.

Finally, they will be able to provide support and information on best practices and appropriate services for neurodevelopmental and mental health needs.
