Home Care Specialist

We are seeking a Home Care Specialist to join our team. This role is ideal for individuals who are proactive, self-directed, and able to work collaboratively with elderly clients.

About the Role:

  • The successful candidate will work with clients and their families to establish goals that are realistic and achievable.
  • You will develop, implement, and evaluate strategies and services that meet the client's needs.
  • Maintaining accurate client documentation is also an essential aspect of this role.
  • You will collaborate with relevant parties to ensure the client's physical, social, emotional, and clinical needs are met.
  • This includes liaising with medical professionals as required.
  • Analyzing the engagement of each client with our organization and suppliers is also a key responsibility.
  • You may be required to participate in our "On Call" system.
  • In addition, you will develop support systems that encourage the involvement of relatives, friends, and community members to the extent defined by the client.
  • You will coordinate care services for clients within specified targets.
