Aged Care Professional

As a dedicated professional in the Aged Care Assessment Team, you will play a pivotal role in ensuring efficient, effective, and timely assessments for clients in both acute care facilities and community settings.
Your primary responsibility will be to conduct comprehensive aged care assessments and provide referrals, support, and information to help clients navigate access to Government-funded aged care services and alternative support options.
