Care Coordinator Registered Nurse

About the Role
We are seeking a highly skilled and compassionate Registered Nurse/Care Coordinator to join our team at Right at Home. As a key member of our care team, you will play a vital role in delivering exceptional care and support to our clients.
Key Responsibilities:
- Building Strong Relationships: Develop and maintain strong connections with clients and their families, ensuring their needs are met and their goals are achieved.
- Care Planning and Coordination: Assign staff to clients based on their qualifications and availability, as well as client preferences, to ensure seamless care delivery.
- Support and Advocacy: Organize support for clients with disabilities, monitor and report on their progress, and refer them to agencies that can provide additional help.
- Assessment and Planning: Assess client needs and resources for health, welfare, and other facilities and services, and develop care plans to meet their unique requirements.
- Communication and Record-Keeping: Respond promptly to emails, enquiries, and requests from clients, staff, and management, and maintain accurate records of incidents and feedback as required.
- Flexibility and Adaptability: Reorganize work priorities to meet the needs of a fast-paced environment, and support all clients to meet their goals.
- Clinical Guidance and Support: Conduct Client Care Assessments and Care Planning, provide clinical guidance to other team members for client management, and discuss and promote Right at Home services with medical professionals and referral sources.
- Requirements:
- Current AHPRA Registration: Possess current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
- Experience and Skills: Have experience in assessing and planning care needs, and possess a strong understanding of the Aged Care and Disability systems.
- Client Focus: Demonstrate a strong client focus, based on their goals, preferences, and needs.
- Compliance and Certifications: Hold a current National Police Check, NSW Working with Children Check, and NDIS Worker Screening Check, and possess a NSW Full Driver's Licence.
- Language and Culture: Be able to communicate effectively in English, and possess a strong understanding of the cultural and linguistic diversity of our clients.
