About the Role:
We are seeking a dedicated and compassionate Care Coordinator to join our Community Team at Anglican Care. As a Care Coordinator, you will play a vital role in supporting our consumers who live in their own homes or visit our day centres.
Key Responsibilities:
- Conduct comprehensive assessments to identify the needs of our consumers and develop personalized care plans.
- Coordinate and deliver care and support services, ensuring that they are safe, effective, and meet the needs of our consumers.
- Work collaboratively with multidisciplinary teams to provide holistic support to our consumers.
- Engage and motivate consumers, families, staff, and stakeholders to promote their well-being and independence.
- Utilize computer and technology skills to manage contemporary systems and maintain accurate records.
Requirements:
- AHPRA Registration as an Enrolled Nurse or Registered Nurse.
- Current drivers licence.
- National Police Check (Anglican Care will consider the relevance of any reported outcomes).
