About the Role
We are seeking a highly skilled and compassionate Registered Nurse/Care Coordinator to join our team at Right at Home. As a key member of our care team, you will play a vital role in delivering exceptional care and support to our clients.
Key Responsibilities:
- Building Strong Relationships: Develop and maintain strong connections with clients and their families, ensuring their needs are met and their goals are achieved.
- Care Planning and Coordination: Assign staff to clients based on their qualifications, availability, and client preferences, ensuring seamless care delivery.
- Support and Advocacy: Organize support for clients with disabilities, monitor their progress, and provide advocacy services as needed.
- Referral and Assessment: Refer clients to relevant agencies for additional support, assess their needs, and provide resources for health, welfare, and other services.
- Communication and Record-Keeping: Respond promptly to client, staff, and management inquiries, maintaining accurate records of incidents and feedback.
- Work Prioritization and Adaptability: Reorganize work priorities to meet the demands of a fast-paced environment, ensuring timely delivery of care services.
- Client-Centered Care: Support clients in achieving their goals, conducting client care assessments, and developing care plans that meet their unique needs.
- Clinical Guidance and Collaboration: Provide clinical guidance to team members, promoting best practices in client management and care delivery.
- Community Engagement: Discuss and promote Right at Home services with medical professionals and referral sources, fostering strong relationships within the community.
- Requirements:
- Current AHPRA Registration: Possess current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency.
- Care Experience: Have experience in assessing and planning care needs, with a strong understanding of the Aged Care and Disability systems.
- Client Focus: Demonstrate a strong client focus, prioritizing their goals, preferences, and needs in all aspects of care delivery.
- Compliance and Certifications: Hold a current National Police Check, NSW Working with Children Check, and NDIS Worker Screening Check, as well as a NSW Full Driver's License.
- Language and Culture: Possess excellent communication and interpersonal skills, with the ability to work effectively with diverse clients and stakeholders.
- Organizational and Time Management: Demonstrate strong organizational, administrative, and time management skills, ensuring seamless care delivery and efficient use of resources.
