Care Coordinator

About the Position

We are seeking a proactive and compassionate professional to join our team and play a vital role in delivering high-quality, person-centred care. The Care Partner will be responsible for coordinating care services under the Support at Home framework, ensuring clients are empowered to make informed choices and maintain independence.

This role offers the opportunity to step beyond traditional case management and into leadership in action. As a key member of our multidisciplinary team, you will work closely with clients, families, and other healthcare professionals to provide holistic support and promote positive outcomes.

Key Responsibilities
  • Deliver person-centred case management tailored to each client's goals and needs
  • Prepare and review budgets with clients to align with their care objectives
  • Manage a caseload, conducting regular assessments and reviews
  • Build and maintain strong, trusting relationships with clients and families
  • Coordinate services across allied health, nursing, and home support
Requirements

To be successful in this role, you will possess strong problem-solving and advocacy skills, as well as a genuine passion for improving the lives of older Australians. You should have previous experience in aged care case management (HCP/CHSP preferred) and a background in community or aged care service coordination.

Additional requirements include excellent planning, communication, and interpersonal skills, with the ability to work independently while contributing to a multidisciplinary team. A valid Australian driver's licence and clean driving record are also essential.
