Our team is seeking a skilled and compassionate Care Manager to lead our clinical operations.The ideal candidate will have a Bachelor of Nursing with current APHRA registration and a postgraduate qualification in aged care. Strong leadership, communication, and interpersonal skills are essential for success in this role.As a Care Manager, you will be responsible for overseeing daily clinical operations, supporting and leading the care home clinical team, and reviewing clinical incidents and complaints.You will also be responsible for managing person-centred care planning and evaluation for residents, including case conferencing with families and carers.Fostering a strong team environment and providing insights and innovative solutions for continuous clinical improvement are critical aspects of this role.Experience in delivering quality clinical care within an aged care or acute setting is highly valued.
  • Key Responsibilities:
  • Oversee daily clinical operations to ensure residents receive the best care available
  • Support and lead the care home clinical team, including coaching, mentoring, staff allocations and delegation, performance management and education
  • Review clinical incidents, complaints and changes, provide solutions to reduce any risks
  • Lead compliance with AN-ACC assessments
  • Manage person-centred care planning and evaluation for residents, including case conferencing with families and carers
  • Foster a strong team environment
  • Prepare and conduct clinical governance audits
  • Provide insights and innovative solutions for continuous clinical improvement
  • Oversee medication management in accordance with resident needs and legislative requirements
  • Liaise with allied health, medical practitioners and other services to provide optimal resident care
Requirements:
  • Essential:
  • Bachelor of Nursing with current APHRA registration
  • Postgraduate qualification in aged care highly regarded
  • Strong and effective leadership, communication and interpersonal skills
  • A person-centred care mindset
  • A passion for nurturing a strong team environment
  • Ingrained passion and empathy for our residents
  • Proven track record in delivering quality clinical care within an aged care or acute setting
  • Excellent clinical assessment and triaging skills as well as strong care planning capabilities
  • Proven and demonstrated experience in fostering a reporting culture balanced with your effective investigative skills
  • An understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes
