Cancer Care Coordinator

Part Time - 48 hours per fortnight

Are you an experienced oncology nurse with a passion for cancer care?

Mildura Base Public Hospital is seeking a skilled Cancer Care Coordinator to provide expert care and support to patients. This role involves coordinating patient care, facilitating multidisciplinary collaboration, and promoting best practices in treatment and education. You will work closely with patients, families, and the Oncology team to enhance patient outcomes, improve efficiency, and ensure continuity of care.

Key Responsibilities:

  • Assess and address the clinical and supportive care needs of cancer patients.
  • Deliver evidence-based care and education, including clinical trial information.
  • Collaborate with the clinical trials unit to recruit eligible patients.
  • Act as a resource for clinical improvement and service innovation.
  • Contribute to the reduction of treatment delays, readmissions, and ED presentations.

Key Selection Criteria:

  • AHPRA registration as a Registered Nurse (Division 1).
  • Minimum 4 years' experience in Oncology or relevant advanced practice.
  • Demonstrated understanding in the psychosocial and clinical needs of patients diagnosed with cancer.
  • Ability to adapt and provide complex information and education effectively to patients' families and carers

This role is remunerated in line with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement as Clinical Nurse Consultant.
