About the Role:

Key Responsibilities:

  • Deliver clinical in-home services, including care plan assessment, medication management, wound care, and chronic disease management.
  • Supervise non-clinical team members in a community setting.
  • Process client intake, develop care plans, and report on client progress.
  • Conduct client care assessments and coordinate the provision of person-centered care according to client needs and service plans.

Requirements:

  • Qualified registered nurse with current registration.
  • First Aid & CPR certification.
  • Australian National Police check.
  • Professional Indemnity Insurance.
