We are seeking a highly skilled and dedicated professional to join our interdisciplinary team as an Access and Engagement Clinician. This is a unique opportunity to work with children and families in a dynamic and innovative program focused on improving health and wellbeing outcomes.
Key Responsibilities- Provide high-quality services to children 0-11 years and their families through skilled triage and assessment, service navigation, care coordination, and consultation.
- Support the team to achieve maximal operational effectiveness and create strong partnerships within and outside of Monash Health.
- Work collaboratively as a member of the Children's Local Service to implement and embed the new program and model of care.
- Commit to continuous quality improvement and patient-centered care.
- Qualification in Speech Pathology, Occupational Therapy, or Social Work.
- Experience in your field, including clinical knowledge in child development and/or mental health.
- Understanding of current, relevant theory, practice knowledge, and evidence in clinical practice.
- Ability to work in an interdisciplinary team and commit to safe best practice and quality care.
- Australian working rights and current driver's license.
