About the Role:

  • Provide focused home-based services for older clientele (65+) with a variety of health conditions, accessing services through their Home Care Package (HCP).
  • Work as part of a large and supportive Occupational Therapy team, with regular team meetings, one-to-one clinical supervision and support from the Clinical lead of Occupational Therapy.

Key Responsibilities:

  • Conduct home safety assessments and provide recommendations for home modifications and equipment.
  • Develop and implement individualised care plans to support clients to achieve their goals and maintain their independence.
  • Work collaboratively with other healthcare professionals to provide comprehensive care to clients.

Requirements:

  • Recognised tertiary qualification in Occupational Therapy.
  • Current registration with AHPRA.
  • Demonstrated knowledge of clinical practice with a variety of health conditions in 65+ year old clientele.
  • Experience working in the Community Aged Care sector with clients receiving services through their Home Care Package (desirable).
