At Outcomes Connect Australia, we deliver best-practice, outcome-focused services to our participants. Our Positive Behaviour Support Model combines the expertise of behaviour support practitioners, occupational therapists, and speech pathologists to understand behaviour drivers from a communication, sensory, and environmental perspective.

About the Role

We are seeking an experienced professional in Shepparton to join our team. You will be responsible for delivering evidence-based, functional behavioural outcomes to clients, including NDIS participants, their families, and carers.

Key Responsibilities
  • Support clients to identify needs through assessment and person-centred planning.
  • Develop, deliver, and evaluate evidence-based support plans in complex support spaces.
  • Ensure individual plans reflect clients' goals, passions, and strengths.
  • Build knowledge, capacity, and strengths of those who support clients – family, carers, friends, etc.
About You
  • You must be registered with the NDIS Commission as a behaviour support practitioner.
  • Demonstrated ability to work positively in a multidisciplinary team.
  • Outstanding communication, influencing, and interpersonal skills.
  • Confident computer skills.
  • Flawless writing skills.
  • Current driver's licence and own reliable vehicle.
