Care Team Facilitator

As a Care Team Facilitator, you will be the first point of contact for rostering and enquiries.

The role involves identifying and establishing care teams; scheduling shift times; responding to daily communication from clients and staff and updating/make changes to rosters.

  • Conduct performance feedback conversations with frontline staff.
  • Assist with developing individual support plans, risk assessments and other administrative tasks.

The ideal candidate will have experience in rostering procedures and ability to work in a fast-paced environment.

Prior experience with Commonwealth Home Support Packages is desirable. The successful candidate will hold or be able to obtain relevant certifications and clearances.
