**Job Opportunity for Community Care Facilitator**

We are seeking a qualified and caring individual to join our team as a Community Care Facilitator. In this role, you will assist older persons from Culturally and Linguistically Diverse (CALD) backgrounds to participate in community life and feel socially included.

The position involves working with frail and older people to develop, maintain or support independent living and social interaction through structured group-based activities.

You will be based in Kilsyth with occasional travel to Narre Warren, Carrum Downs and other locations in Southern Metro Region.
